Supported Housing Program

The Supported Housing Program (SHP) provides rental assistance to individuals diagnosed with mental illness. The purpose of the program is to help individuals return to independent living within Nassau County. Case managers assist individuals locate and secure housing. Individuals are responsible to contribute 30% of their income. Participants in the program receive support services including independent living skills.

Program Requirements:

Individuals must have a diagnosis from a certified psychiatrist from the State of New York. Priority is given to Nassau County residents. Must be able to live independently and contribute 30% to the rent. Please contact office for forms, application and details.
